Patients complete forms on their phone before they arrive. Answers land in the OpenDental chart as structured data, allergies as allergies, medications as medications.

What it does instead.
Build the form you actually use
A drag-and-drop builder with dental-specific blocks, medical history grids, allergy and medication pickers, consent with signature.
Structured, not scanned
An allergy becomes an allergy record. A medication becomes a medication record. Not an image of a page nobody opens.
Sent by text, opened on a phone
One link per request, single-use, verified against the patient's date of birth. It expires after it's submitted.
Honest sync status
Molar tells you exactly what made it into the chart and what didn't. It never reports a write it can't confirm.
How it syncs.
Form answers map onto real OpenDental records, and the mapping is verified rather than assumed.
- Demographics, allergies, diseases and medications write to their own OpenDental records, proven against a live install, not inferred from documentation.
- Each write is read back; the status you see is computed from what OpenDental returned.
- Anything that can't be mapped is flagged for review instead of being silently dropped.
Questions we get asked.
Do patients need an app or a login?
No. They get a text with a link, confirm their date of birth, and fill the form in their phone's browser.
What if a patient doesn't fill it out?
You can see who hasn't, and they can still fill it in the waiting room on their own phone.
Can we use our existing forms?
You can rebuild them in the builder, and most practices trim them in the process. We'll go through yours on the demo call.
What about signatures and consent?
Signature capture is built in, stored with the submission and available on the chart.
